Every applicant for a license to maintain or operate an X-ray laboratory shall make application in a blank form which shall be furnished by the Department of Health, and on said blank there shall be printed all the requirements necessary for the installation and operation of said X-ray laboratory, guaranteeing the public due protection against danger from the electric current used.
