Every application for a license shall be filed with the Department of Health upon forms provided by it and shall contain such information as said Department may reasonably require, which may include affirmative evidence of ability to comply with such reasonable standards, rules and regulations as are lawfully prescribed hereunder. Each application for a license for health facilities whose annual operating expenses are not derived in at least seventy-five percent (75%) from public funds must be accompanied by a license fee which shall be returned in full if the license is denied. The initial license and renewal fees for the facilities defined in this chapter shall be on the basis of five dollars ($5) per authorized bed.These sums shall be covered into the Health Fund created by § 337j-1 of this title.
