The licenses referred to in §§ 91—91i of this title are to be applied for to the Secretary of Health by the person having established and in operation, or proposing to establish and operate in the future, a clinical analysis laboratory, a plasmapheresis center, a seropheresis center or blood bank. Applications for license shall be made on the printed forms to be furnished by the Department of Health. Licenses granted under §§ 91—91i of this title shall be issued for the specific establishment and shall be the responsibility of the director and of the owner to operate it in accordance with §§ 91—91i of this title and with the regulations promulgated hereunder.Said licenses shall be granted for periods of one (1) year; Provided, That same may be revoked, cancelled or denied at any time by the Secretary of Health, if any provision of §§ 91—91i of this title or of the regulations promulgated thereunder is violated.
