Every school, daycare center, or social treatment center shall keep an immunization record of every student or preschool child. The immunization record form shall be the same for all the schools, daycare centers or social treatment center, provided by the Department of Health. This immunization record shall be open to inspection by the agents authorized by the Department of Health. The immunization record of each student or preschool child shall include the name of the student, date of birth, social security number, medicaid number, if applicable, admission date, provisional admission date, if such is the case, type of vaccines or toxoids administered and the date each vaccine was administered, date and type of exemption if any. When a student or preschool child is transferred from one school to another, or from a daycare center, or social treatment center, his record of immunization shall be included with his student record, to the institution to which he has been referred.
