The registrar, or the director of the social treatment center, or the director of the daycare center shall file a report to the Department of Health within the next sixty (60) calendar days after the start of the school term, or the enrollment of the student or preschool child. The report shall be prepared on the forms supplied by the Department of Health and shall indicate the number of students admitted to the school, social treatment center and daycare center who have an immunization certificate; the number of students exempted as provided in § 182d of this title, and those that have been provisionally admitted as provided in § 182c of this title.
