Every institution that provides ambulatory health services shall have a descriptive services manual, which shall include, as a minimum, the following:(a) The criteria of admission that must be met any person who requests the services.(b) The age and level of care of the applicants.(c) The model of the individualized treatment, recovery and rehabilitation plan.(d) The composition of the personnel the institution shall have, as well as their qualifications; Provided, That this section shall not apply in those cases of private mental health professional offices.
