Every limited health service organization shall pay to the Commissioner the established fees for the following:\n(a) For filing an application for a certificate of authority or amendment thereto;\n(b) for filing a material modification of or an addition to a limited health service;\n(c) for filing each annual report, and\n(d) for filing periodic reports as required by the Commissioner.\nHistory —Aug. 29, 2011, No. 194, added as § 16.240 on Aug. 23, 2012, No. 203, § 2, eff. 90 days after Aug. 23, 2012.
