In the event of theft, loss, mutilation or destruction of a license or certification, an optometrist may petition the Board for a new one by submitting a statement or a copy of the complaint filed before the police of Puerto Rico, that confirms that he or she and no other person was the legal possessor of the stolen, lost, mutilated or destroyed license. The optometrist shall include together with this statement or copy of the complaint, the corresponding payment for a duplicate license and any other document that the Board may require pursuant to regulations.
