The Board shall collect the following fees through a bank or postal money order, or a certified check drawn to the order of the Secretary of the Treasury:\nOccupational Therapists :\nFor revalidation examination $ 25.00 For license $ 30.00 For reexamination $ 20.00 For reciprocity license $ 50.00 For duplicate of lost or misplaced license $ 25.00 For recertification and registration $ 30.00 For provisional license $ 50.00 Assistant in Occupational Therapy : For revalidation examination $ 15.00 For license $ 25.00 For reexamination $ 15.00 For reciprocity license $ 40.00 For duplicate of lost or misplaced license $ 25.00 For recertification and registration $ 25.00 For provisional license $ 20.00\nThe fees collected by the Board cannot be returned under any circumstances.
