The fees indicated hereinbelow shall be paid through a postal or bank money order, or certified check in the name of the Secretary of the Treasury:Revalidation examination $ 35.00 License $ 75.00 License through reciprocity $ 75.00 Provisional license $ 25.00 Duplicate license $ 15.00 Recertification and registration $ 75.00The funds generated on account of the fees set in this section shall be covered into the Health Fund, for the use of the Office of Regulation and Certification of Health Professionals of the Department of Health.
