§ 14-1-12

(a) The petition shall plainly state the facts which bring the child within the provisions of this chapter, and shall set forth the name, age, and residence of: (1) The child; (2) The child’s parents; (3) The child’s other legal guardian, if there is one; (4) The person or persons having custody or control of the child; and (5) The nearest known relative, if no parent or guardian can be found. (b) If any of the facts required by subsection (a) of this section are not known by the petitioner, the petition shall so state.
