(a) Application for certification shall be made on forms supplied by the board. (b) The application shall be accompanied by a fee of ten dollars ($10.00). The annual renewal fee will be five dollars ($5.00). (c) The board shall review applications and supporting documents, determine the eligibility of the applicant for examination, and notify the applicant of the same.
