Within ten (10) days after completion of a new or replacement well, a well drilling contractor shall provide the owner, the building official and the department of health a well installation report conforming to the form, content, and means specified by the department of health. This well installation report will at a minimum indicate: (1) The well owner’s name and address, (2) The physical location of the well, (3) The well depth, (4) The geologic materials and thickness of materials penetrated, (5) The length and type of casing, (6) The static water levels, (7) The results of a well yield test that conforms to industry standards, and (8) Any other additional information which may be required by the department of health.
