§ 5-35-1-2

Every applicant for licensure shall present satisfactory evidence in the form of affidavits properly sworn to that he or she: (1) Is of good moral character; and (2) Has graduated from a school or college of optometry that maintains a course in optometry of not less than four (4) years and is approved by the Accreditation Council on Optometric Education (ACOE) or other accrediting body as approved by the board; and (3) Has successfully passed a national examination or other examinations approved by the department and the board; and (4) Any other qualifications as may be established in rules and regulations promulgated by the department.
