Greeting:You are hereby notified that you have been drawn as a juror for the superior or family court for the county (or counties) of ......................... and you are required to attend the said court be holden at ......................... on the ......................... day of ......................... , at ......................... o'clock in the forenoon. Constable.The jury commissioner or his or her agents, having retained the notifications in accordance with § 9-10-1, shall forthwith serve the notifications in the same form as contained in this section by regular mail.
