(A) Every health maintenance organization subject to this chapter shall pay to the department the following fees: (1) for filing an application for a certificate of authority, two thousand dollars; (2) for filing an amendment to the organization documents that requires approval, one hundred dollars; (3) for filing each annual report, one thousand dollars; (4) for transferring a certificate of authority from one entity to another which qualifies for such a certificate of authority, two thousand dollars. (B) Fees charged under this section must be deposited in the general fund of the state. Fees required in this section must be fully earned when paid and are not refundable, proratable, nor transferable.
