3-1A-5. Forms provided--Value not required--Verification--Open to public inspection.The secretary of state shall prescribe and provide forms for the reporting of close economic interest. The value of a close economic interest need not be reported. Each individual filing a statement of financial interest shall subscribe to an oath or affirmation verifying the contents of the statement.
