55-11-8. Information to be submitted upon registration.When a person registers to sell prearranged funeral trust contracts, he shall submit to the State Board of Funeral Service the following information:(1)The name and address of the funeral establishment or cemetery association which has authorized him to sell prearranged funeral trust contracts;(2)His name and address;(3)A sworn statement that he has not been convicted of any crime within the last five years involving the elements of fraud or misrepresentation; and(4)A statement of compliance with chapter 37-13.
