58-41-8. Copies of forms required with application.Each application for a certificate of authority shall include:(1)A copy of the form of each evidence of coverage to be issued to the enrollees;(2)A copy of the form of each individual or group health maintenance contract which is to be issued to enrollees or their representatives.
