Section 12-4-306 - Forms for reports and statements

All forms for reporting figures by health care providers shall be designed and specified by the department of health, in consultation with the comptroller of the treasury and the state departments purchasing health care services and the Tennessee Hospital Association. All providers shall be required to submit statements on the form or forms prescribed and designed by the department for this purpose.Acts 1961, ch. 117, § 6; T.C.A., § 12-430; Acts 1984, ch. 787, § 6.
