Section 45-12-105 - Contents of written application for license

Each application for a license shall be in writing and made under oath or affirmation to the commissioner, in a form prescribed by the commissioner, and shall include the following:(1) The legal name, residence and business address of the applicant and, if the applicant is a partnership, association, or corporation, of every member, officer, managing employee and director thereof;(2) The location in this state where the registered agent of the applicant shall be located; provided, that 'registered agent of the applicant' includes a person designated by the applicant for accepting notices or orders by the commissioner, or process affecting the applicant, pursuant to § 45-12-103; and(3) Other data and information the commissioner may require with respect to the applicant, and its directors, trustees, officers, members, managing employees or agents.Added by 2014 Tenn.
