Every prepaid limited health service organization subject to this chapter must pay to the department the following fees:(1) For filing an application for a certificate of authority: five hundred dollars ($500);(2) For filing an amendment to organization documents that require approval: fifty dollars ($50.00);(3) Each annual report: one hundred dollars ($100); and(4) For each renewal of certificate of authority each year: one hundred dollars ($100).Acts 2000, ch. 948, § 45.
