A fee as set by the board will be charged for:(1) Replacement of any lost, misplaced or mutilated license; or(2) Change of name or mailing address by any cosmetologist, aesthetician, manicurist, natural hair stylist or instructor licensed under this part.Amended by 2024 Tenn.
