§ 8801.156

(a) A person must submit an application to the board to obtain a permit under this chapter.(b) The application must state:(1) the name and address of the person requesting the permit;(2) the location and wellhead elevation of the well or proposed well;(3) the amount of water being withdrawn or proposed to be withdrawn; and(4) any other information necessary for the board to control and prevent subsidence in the district.(c) The board shall set a reasonable fee for processing an application.
