§ 8834.207

(a) A person must submit an application and an application fee to the board to obtain a permit under this chapter.(b) The application must state:(1) the person's name and address;(2) the location and wellhead elevation of the well or proposed well;(3) the amount of water being withdrawn or proposed to be withdrawn; and(4) any other information required by the board as necessary for the board to control and prevent subsidence in the district.(c) The board shall use the application fee to process applications.
