(a) Except as provided by Subsection (b), a permit under this subchapter must include:(1) the name of the applicant;(2) the date of issuance;(3) the signature of the director of the department;(4) a statement of the kind of equipment to be transported over the highway, the weight and dimensions of the equipment, and the kind and weight of each commodity to be transported; and(5) a statement of any condition on which the permit is issued.(b) A permit issued under Section 623.071(a) or (b) must also state:(1) each highway over which the equipment is to be transported, if the permit is for a single trip; or(2) the region or area, as required by rule, over which the equipment is to be operated, if the permit is for other than a single trip.
