(a) The applicant shall include on the application the applicant's:(1) driver's license number or the number of a personal identification card issued to the applicant under Chapter 521;(2) military identification number; or(3) driver's license number of a driver's license issued by another state or country if the applicant is not a resident of this state and is seeking medical treatment in this state. (b) The county assessor-collector shall record on any disabled parking placard issued to the applicant the following information in the following order:(1) the county number assigned by the comptroller to the county issuing the placard;(2) the first four digits of the applicant's driver's license number, personal identification card number, or military identification number; and(3) the applicant's initials.
