The certificate shall be valid for five years from date of issuance. The Commissioner shall establish the following by rule: (1) procedures for issuing a certificate under this section;(2) qualifications required for certification under this section;(3) privileges granted to a person holding a certificate under this section.(b) Fees to be charged to a person applying for certification under this section shall be: (1) Application fee $ 25.00 (2) Initial certification—resident $100.00 (3) Initial certification—nonresident $200.00 (4) Renewal—resident $125.00 (5) Renewal—nonresident $225.00 (Added 1995, No. 65 (Adj. Sess.), § 1; amended 1997, No. 155 (Adj. Sess.), § 47, eff. Jan. 1, 1999.)
