Forms The Secretary of State shall formulate forms for the returns and shall, on request, furnish such forms by mail or otherwise to persons, copartnerships, associations, or corporations subject to the provisions of this chapter. (Amended 1961, No. 217, § 1, eff. July 13, 1961.)
