Informed consent A licensed midwife shall provide each client with and maintain a record of a signed informed consent form that describes the midwife’s education and credentials, whether the midwife has professional liability insurance coverage, procedures and risks of home birth, a copy of the emergency plan required by section 4190 of this title, and the address and phone number of the Office of Professional Regulation where complaints may be filed.
