Applications for license shall be on the form specified by the Commissioner, shall be sent to the Commissioner and shall be accompanied by satisfactory proof that the principal sponsor of the health facility or health service:\n(1) is twenty-one years of age or over;\n(2) is of good moral character as shown by at least two letters of favorable character testimony; and\n(3) is not addicted to intemperate use of alcoholic stimulants or narcotic drugs. The application shall contain such information as the Commissioner reasonably requires, which may include affirmative evidence of ability to comply with such reasonable standards and regulations as are lawfully prescribed under the provisions of this chapter.
