The purpose of this subchapter is to facilitate the discharge of patients who require long-term caregiver services, provide notice to, and involve caregivers in the hospital’s discharge plans for the patient, and assist patients and their caregivers by providing discharge plans that effectively explain and instruct in all after-care tasks necessary to maintain the patient’s ability to reside at home.
