A guardian shall promptly notify an incapacitated person’s closest relatives and any person designated by the incapacitated person to be notified, along with the appropriate contact information and upon the incapacitated person’s knowledge of such circumstance and information, if:\n(1) The incapacitated person’s residence has changed;\n(2) The incapacitated person is staying at a location other than the incapacitated person’s residence;\n(3) The incapacitated person is admitted to a medical facility for:\n(A) Emergency care in response to a life-threatening injury or medical condition; or\n(B) Acute care; or\n(4) The incapacitated person dies. However, notification of the incapacitated person’s death must be made in person or by telephone.
