Whenever the Commission revokes or suspends a license issued pursuant to this chapter, it shall:\n1. Enter an order to that effect, which order shall set forth its findings;\n2. Compile a record containing (i) a summary of the evidence, (ii) the findings with respect thereto, (iii) the order, and (iv) the reasons supporting the revocation or suspension; and\n3.
