(a) The Bureau shall prescribe the written and electronic forms for reporting all required information regarding patients with tuberculosis.(b) The Bureau shall prescribe the written and electronic forms to be used in committing patients to any state hospital or other health care facility where care and treatment of tuberculosis patients is conducted.
